Release date: Mon 19 December 2011

Last updated: Thu 5 July 2018

Personal details

If you have a firearm licence or permit, you are required by law to notify Victoria Police's Licensing & Regulation Division in writing within 14 days of any changes to:

  • The name that appears on your licence or permit
  • The address which appears on your licence or permit
  • Your permanent place of residence
  • You postal address
  • The address where any firearm held under the licence is ordinarily stored
  • The address where you carry out your business if you are a licensed firearm dealer

You are breaking the law if you fail to do so.

Change of address: Where any address details have changed - residential, postal, business or ordinary storage - you are required by law to inform Licensing & Regulation Division of this change within 14 days. To do this, you can email, post or fax a completed Change of Details form (PDF) to LRD.

Requests for change of address can also be submitted to LRD in writing via letter, email or fax. Please remember to provide the following details in your request:

  • your full name
  • your licence number; and
  • confirmation of whether your postal and ordinary storage address have also changed.

Once your request has been processed, LRD will send you an adhesive label printed with your new address that you will need to attach to the back of your plastic licence.

Change of name: You are required by law to inform Licensing & Regulation Division of your change of name. To do this, you must post the completed Change of Details form to LRD, along with a certified copy of:

  • the change of name certificate issued by the Victorian Registry of Births, Deaths and Marriages; OR
  • your full birth certificate (including the schedule of all names changes); OR
  • your marriage certificate; OR
  • your divorce certificate.

Only certified copies will be accepted. A certified document is a copy of a document or record signed and certified as a true copy of an original by an authorised person. The person certifying a document must sight a copy of the original at the time they are certifying the copy.

Once your request has been processed, LRD will send you a payment notice for a duplicate licence in your new name.

You will then need to take proof of identification and proof of payment to an approved photo point. LRD will not accept passport (or other) photos that have not been taken by an official at an authorised photo point unless you have completed this form declaring you cannot attend a Victorian photo point (PDF).

You must notify Licensing & Regulation Division in writing as soon as practicable if your plastic licence is lost, stolen or damaged.

You will need to email, post or fax a completed Request for Duplicate Licence form (PDF) so LRD can send you a new payment notice. You will then need to take proof of identification and proof of payment to an approved photo point .
