Release date: Wed 31 January 2018

Last updated: Tue 30 January 2018

Before submitting your request for your forensic sample, please read the following frequently asked questions:

How do I fill out the online form?
Select either the Victoria Police Online Forensic Sample Request Form - Preferred Method or the Victoria Police Online Forensic Sample Request Form - Alternative Method and complete the required fields. Save a copy of the completed form to your computer, then attach the file to an email (along with copies of your 100 points of identification) and send to the Road Policing Drug and Alcohol Section via email. You can also print copies of your completed form and identification documents and send via post. (See contact details at bottom of page).

What will happen if I do not want to, or cannot, complete all the required details in the online form?
Unless Victoria Police can confirm your identification and match it to a forensic sample, Victoria Police will be unable to authorise the release of your sample. You will need to contact Victoria Police via email or post (see contact details at bottom of page) stating your circumstances. All required fields in the Victoria Police Online Forensic Sample Request Form (Preferred Method and Alternative Method) are marked with an asterisk.

Can I disregard the online form and contact the Victorian Institute of Forensic Medicine (VIFM) directly to arrange collection independently of Victoria Police?
No, Victoria Police must first confirm identification before allowing VIFM to release forensic samples.

Can I have my sample analysed by VIFM?
No, this would be a conflict of interest.

Can I request a copy of the toxicology results directly from VIFM?
No.

Can Victoria Police recommend a Pathology Unit to test my forensic sample?
No, this would be a conflict of interest.

I live in regional Victoria. Can Victoria Police post my forensic sample to me?
No, you can only choose from the two collection methods provided by Victoria Police. If you select the Alternative Method, samples may only be collected from the Melbourne West Police Station at 313 Spencer Street, Docklands, 3008, by the person you nominated on your Victoria Police Online Forensic Sample Request Form (yourself, a nominated legal representative or third party) at the prearranged date and time set by Victoria Police. If you chose the Preferred Method, the sample can only be collected from VIFM by the courier nominated by your Pathology Unit.

How long will this process take in its entirety?
Unfortunately we are unable to provide a time frame for this process.

Can you provide information to my insurance company relating to my forensic sample or toxicology results?
No, for privacy reasons we are unable to liaise with your insurance company.

I requested a blood sample after a breath/oral fluid test. How do I obtain a part of the sample?
If you were not handed a part of your sample at the time it was taken, please use one of the two methods provided. This process relates to any sample taken under the Road Safety Act 1986, Rail Safety (Local Operations) Act 2006 or Marine (Drug, Alcohol and Pollution Control) Act 1988.

A member of Victoria Police requested a blood sample in lieu of an evidential breath/oral fluid test. How do I obtain a part of the sample?
Follow one of the two methods provided. This process relates to any sample taken under the Road Safety Act 1986, Rail Safety (Local Operations) Act 2006 or Marine (Drug, Alcohol and Pollution Control) Act 1988.

A member of Victoria Police requested a forensic sample after a Drug Impairment Assessment. How do I obtain a part of the sample?
Follow one of the two methods provided. This process relates to any sample taken under the Road Safety Act 1986, Rail Safety (Local Operations) Act 2006 or Marine (Drug, Alcohol and Pollution Control) Act 1988.

Can someone other than the person I have nominated on the Victoria Police Online Forensic Sample Request Form collect the sample on my behalf?
No, the sample can only be released to the individual nominated on the Victoria Police Online Forensic Sample Request Form. You will have to resubmit the form if you wish to make changes to the person nominated to collect your forensic sample.

If for any reason I resubmit the Victoria Police Online Forensic Sample Request Form, but had already been given a collection time and date, will the date/time/location remain the same?
No, you will be given a new time and date. The location will remain the same in these circumstances.
